Here's the form for your activity write up. There are some additional suggestions on the first page to help you out. Also pay attention to the instructions on submitting your form to the intersession committee.
Here's the spreadsheet for ideas of what you think you're going to do. This will help the committee get a sense of what's out there, the cost ranges, etc. You will need to be logged into your google account to access this.
